Academic and/or Financial Aid Suspension Appeal

Students who did not meet the minimum qualifications to receive financial aid may request an appeal. The financial aid suspension appeal will be considered by the Financial Aid Appeals Committee.

American Indian Scholars Program (AISP)
Used by eligible members of federally recognized American Indian Tribes, Canadian First Nations, and Minnesota Tribes to apply for tuition and fee-free pathway funding.

Foster Youth 
Current and former foster youth can get up to $5,000 per school year for post-secondary education.

Minnesota Post Secondary Child Care Grant
Used by Minnesota resident students to apply for child care funding

Veteran Enrollment Request to Certify Form
Used by students who wish to request the use of their Veteran Education Benefits. Students must be registered for the classes for the semester in which they wish to receive benefits before completing this form.

College Consortium Agreement

Used when an LSC degree-seeking student wishes to take classes at another college and have those credits count toward their total enrolled credits for financial aid purposes.

Declining the MN North Star Promise

Use this form to decline the Minnesota North Star Promise award. May be done when a student who is eligible for that program is also eligible to receive another last-payer tuition benefit, such as Post-9/11 GI Bill (Chapter 33), which will be negatively impacted by receiving North Star Promise. May also be used by students who are receiving only a small award amount from North Star Promise who wish to save their program eligibility (limited to 60 credits at a 2-year college, 120 credits at a 4-year college) for another point in their academic career.

Used by students who wish to have LSC release information to others.
